You are an exciting and fun part of our practice! We have implemented these credits as an incentive to encourage our patients to develop good dental habits and have some extra fun during their treatment. These credits are redeemable at our prize gallery which includes a variety of prizes for all ages. You may also elect to donate these credits to a selected charity.

You will earn credits each time you complete the following:

• Keep your regularly scheduled appointment and be on time… 1 credit
• Good Oral Hygiene, your teeth should be clean when you are seated… 1 credit
• Nothing loose or broken (this means brackets, bands, removable appliances, etc.)… 1 credit
• Wear your Dr. JAW t-shirt to your appointment… 1 credit
• Follow all instructions regarding wearing your rubber bands, headgear, etc… 1 credit
• If Dr. Weaver or any of our staff see you outside the office with your t-shirt on… like the mall, movies, basketball games, etc… 5 credits
• Get your teeth cleaned at your regular dentist’s office. 5 credits

Rules

1. Credits are only awarded at regularly scheduled appointments. Walk-in or Emergency visits do not apply.

2. The maximum number of credits you can receive is 150.

3. The number of credits you earn at each appointment is up to you.

4. The credits prize board is located near the front desk checkout area.
The prizes may change from time to time. Not all prizes will always be available.

5. Your credits are your responsibility. We will write the number of credits earned at each appointment on your next appointment card.  Keep all your appointment cards to redeem your credits.  The credits are just like real money; if you lose them, like money, they are gone!

6. Credits are not transferrable to any other patient.  They are yours and yours only.

7. Once you spend your credits on a prize, they are spent. You must earn more credits to purchase more prizes.
